📝 Step-by-Step Instructions

1. Receive Your Kit

  • Your kit arrives in a brown box. Inside is a white box—keep this box as it includes your return shipping label.
    (Your phlebotomist will handle the return shipping for you.)

2. Find Your Test Requisition Form

  • Inside the white box, locate the requisition form.
    This form is for your phlebotomist and includes a QR code to help you find a local lab or mobile phlebotomist.
    If you need assistance, contact your Client Care Coordinator.

3. Prepare the Supplies

  • The box contains everything your phlebotomist will need.
    Do not open or handle the tubes yourself.
    Freeze the included ice packs before your draw day—these will be used for return shipping.

4. Follow Pre-Test Guidelines

  • Fasting: Unless instructed otherwise, fast 8–10 hours before your draw. Water, black coffee, or tea (no sugar/cream) are allowed.
    Supplements/Medications: Avoid taking them for 2 days before and the morning of your draw.
    Testosterone: Schedule your draw at trough (day before next injection).
    Hormone Creams: Do not apply the morning of your test.
    Menstruating Females (Hormone Panel): Aim for day 19 of cycle, or let us know if missed.
    If sick or post-vaccine: Wait 2–3 weeks after recovery.

5. Day of Your Test

  • Bring your full kit, including frozen ice packs, to your lab or mobile phlebotomist.
    Ensure your phlebotomist reviews the processing instructions before starting.

6. After Your Draw

  • Your phlebotomist will process, label, and pack everything into the white box.
    They will use the pre-labeled FedEx package to ship your samples.

7. Notify BLI

  • Let your Client Care Coordinator know via the patient portal that your labs are complete.
    Typical turnaround time: 3–4 weeks.
